Date: required; select today's month and day

Email Address: required; double check for correct spelling

Name: required; fill in completely first and last name

Farm/Company Name: if you have a company farm name fill it in

Mailing Address: required; fill in the appropriate boxes for your complete mailing address

Phone: required; fill in your complete daytime phone number - include the area code

Fax: fill in your complete fax number including the area code
Step 2
Number of acres to be Composite Sampled: fill in the acres to be composite sampled (Package 1 - 1 core per acre) (Package 2 - 1 core per two acres)

Number of fields to be be Sampled: fill in how many fields are to be sampled
Step 3
Calculated Field: based on the acres to be sampled (step 2) the appropriate price will be automatically calculated here
Step 4
Calculated Field: based on the number of acres (step 2) the minimum number of samples to be analyzed is calculated here. You may insert a larger number if you wish to have more samples analyzed.
Step 5

Optional Add Ons -
Micros: if you wish Micros testing enter the number of samples to be tested.

Boron: if you wish Boron testing enter the number of samples to be tested.

Step 6
Additional: Analysis Manure, Tile Water, Plant Tissue
You may add any of these analysis to your package. Fill in the number of samples in the appropriate boxes.
